VitalAir provides care without discrimination, and this notice explains your rights.
This practice complies with applicable federal civil rights laws and does not discriminate, exclude people, or treat them differently on the basis of the characteristics listed below. Effective August 2026.
Our Policy
We do not discriminate on the basis of race, color, national origin, age, disability, or sex, including sex characteristics, sexual orientation, gender identity, and pregnancy or related conditions. This applies to every part of our care and services, including who we accept as patients, how we schedule and deliver care, and how we handle billing.
Support for People With Disabilities
We provide reasonable modifications and free aids and services to help people with disabilities communicate effectively with us. Depending on what you need, this can include qualified sign language interpreters and written information in other formats, such as large print or accessible electronic formats.
Support for People Whose Primary Language Is Not English
We provide free language assistance services to people whose primary language is not English, including qualified interpreters and information written in other languages. There is never a charge to you for this, and you are never expected to bring your own interpreter.
How to Request These Services
Call the practice and tell us what you need, ideally before your visit so we can arrange it in advance. If arranging it in advance is not possible, tell us when you arrive and we will make arrangements then.
Filing a Grievance With the Practice
If you believe we have failed to provide these services, or have discriminated against you in any of the ways described above, you may file a grievance with the practice using the contact details at the end of this notice. You may file in person, by mail, by phone, or by fax, and we can help you complete the grievance if you need assistance. Filing a grievance will not affect your care.
Filing a Complaint With the Office for Civil Rights
You may also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, and you may do so at the same time as filing with us. Complaints can be filed electronically through the OCR complaint portal, or by mail or phone. Current filing instructions and contact details are published at hhs.gov/ocr.
